The MGC pool, tennis courts, clubhouse and greenspace (MGC facilities)  are for the use of all active and in good standing members and their guests. Members and guests are expected to follow policies and rules, act responsibly, and use good judgment in use of the facilities. The MGC and Board of Directors assume no responsibility or liability for any accident or injury of any person or damage or loss of personal property on any MGC property. The MGC Board of Directors reserves the right to add, delete or modify these rules when necessary.

PLEASE READ CAREFULLY, BY SIGNING THIS DOCUMENT, YOU CHOOSE TO WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.

To: Meadowgrove Club

I/we (the whole family) , __________________________, wish to use Meadowgrove Club facilities. By using the Meadowgrove Club facilities, I/we recognize and fully understand certain things, including:

• Using MGC Facilities, you (the /member/your guest) is/are responsible for the safe use of the greenspace/clubhouse and tennis courts.
• Swimming at Meadowgrove Club, you (the swimmer/member/your guest) is/are responsible for the safe use of the pool.
• I/we understand, when a lifeguard is present, I/we S.A.Y.O.R is still enforced.
• My/our (the swimmer) use of the pool facilities may involve certain risks including but not limited to:
• The risk of injuries resulting from possible malfunction of pool equipment.
• The risk of injuries resulting from tripping or falling over obstacles in the pool area.
• The risk of injuries resulting from unsupervised swimmers colliding.
• The risk of other injuries resulting from participating in any action in the pool.
• I/we recognize and fully understand that the above list is not a complete or exhaustive list of all possible risks; the list only provides examples of types of risks that I/we are assuming. In exchange for the MGC facilities allowing me/us to utilize the pool/greenspace/clubhouse/tennis courts. I/we hereby agree to the conditions below. 
• I/we fully read and understand the rules and policies for the pool and tennis courts.
• I fully intend and choose to give up the legal rights as stated below:
• TO WAIVE ANY AND ALL CLAIMS that I have or may have in the future against the Meadowgrove Club, its directors, officers, employees, lifeguards, agents, or representatives (hereinafter referred to as the “MGC”) relating to my/our use of the pool and pool area.
• TO RELEASE THE MGC from any and all liability for any loss, damage, injury, expense, or other cost that I/we may suffer or that my next of kin may suffer in connection with my/our use of the MGC facilities to any cause whatsoever, INCLUDING NEGLIGENCE ON THE PART OF THE MEADOWGROVE CLUB.
• TO HOLD HARMLESS AND INDEMNIFY THE MGC from any and all liability to property, or personal injury to, any third party, resulting from the use of the MGC facilities.
• That I/we are over the age of 18 or that if I/we are under the age of 18 my parent/guardian who is a member of Meadowgrove Club, in good standings with fees, has signed this form and that I/we are responsible and will adhere to all the rules of the MGC facilities.
• That this Waiver, Release, and Agreement is fully effective and shall be effective and binding upon me/us, and my heirs, next of kin, executors, administrators, and assigns, or anyone else authorized to act on my behalf or on behalf of my estate.

I/we have read and understood this document. I/we are aware that by signing this document, (clicking the box on my membership application as my signature) I am waiving certain legal rights that I/we may have against the Releases, and I/we fully agree to do so.
I/we also acknowledge that should I/we (member-s) be doing anything considered inappropriate or disorderly, or disturbing to other swimmers I/we may be asked to leave by a member of the MGC/lifeguard. Should multiple complaints be made it will be required that I/we (the member-s) won’t be allowed to use Meadowgrove Club Facilities.
